Chronic Care Management & Let's Beat Diabetes and Open Day, November 2007
On 6 November 2007, Counties Manukau DHB held a national open day for all DHBs, PHOs and Ministry staff interested in Chronic Care Management (CCM) and Let’s Beat Diabetes (LBD). The key objective of the day was to share our experiences in design, implementation, support and evaluation.
