Advance Care Planning

National Advance Care Planning Cooperative

Counties Manukau DHB is an active member of the National Advance Care Planning Cooperative. The Cooperative was formed in June 2010 by clinicians from across the country which aims to ensure that Advance Care Planning meets the needs of all New Zealanders.  The Cooperatives vision is that “All people in New Zealand will have access to comprehensive, structured and effective Advance Care Planning”.  The Cooperative will work together to develop a common understanding, framework and direction for Advance Care Planning in all areas of health, for our communities. Counties Manukau will continue to work with the Cooperative to achieve this aim.

The National Advance Care Planning Cooperative has developed a number of Advance Care Planning documents for the general public. This material includes an Advance Care Planning guide and an advance care plan. While a number of providers from across the country have transitioned to this material CMDHB will continue to use its own Advance Care Planning documents which are in a use in some secondary settings where Advance Care Planning has been implemented.  As CMDHB is the only DHB to have developed a process which enables an advance care plan to reach our patient information service electronically, it is important that we continue to use our Advance Care Planning documents for this reason.
